ABSTRACT Objective This study aimed to investigate the association between depressive symptoms and food insecurity in households with older adults. Methods This is a cross-sectional, quantitative study conducted with community-dwelling older adults attended to in the Family Health Strategy in a municipality in the Brazilian Northeast. Depressive symptoms were evaluated using the Geriatric Depression Scale and food insecurity was assessed using the Brazilian Food Insecurity Scale. The chi-squared test was applied for a bivariate analysis and binary logistic regression was used to verify the association between depressive symptoms and food insecurity, adjusted for potential confounding variables. The significance level was p<0.05. Results A total of 316 older adults were evaluated, with a mean age of 70.5 (±7.5 years). The prevalence of depressive symptoms was 27.5% and that of food insecurity was 63.3%, with 25.6% of households with older adults experiencing moderate/severe insecurity. In the multivariate analysis, households experiencing mild food insecurity presented 3 times (OR: 3.02; 95% CI: 1.42-6.39) more chance of developing depressive symptoms, while in those experiencing moderate/severe food insecurity the chance was 5 times higher (OR: 5.01; 95% CI: 2.30-10.92). Conclusion An association was found between food insecurity and depressive symptoms in households with older adults of the Family Health Strategy, with more chances for those experiencing moderate/severe food insecurity.


RESUMO Objetivo Investigar a associação entre sintomas depressivos e insegurança alimentar em domicílios com idosos. Métodos Estudo transversal, quantitativo e realizado com idosos comunitários atendidos na Estratégia Saúde da Família em um município do Nordeste brasileiro. Sintomas depressivos foram avaliados pela Escala Geriátrica de Depressão; e a insegurança alimentar, com a Escala Brasileira de Insegurança Alimentar. Foi aplicado o teste qui-quadrado para análise bivariada e a regressão logística binária para verificar a associação entre sintomas depressivos e insegurança alimentar, ajustada por potenciais variáveis de confundimento. O nível de significância foi p<0,05. Resultados Foram avaliados 316 idosos, com média de idade de 70,5 (±7,5 anos). A prevalência de sintomas depressivos foi de 27,5% e a de insegurança alimentar foi de 63,3%, sendo que 25,6% dos domicílios com idosos estavam em insegurança moderada/grave. Na análise multivariada, domicílios com insegurança alimentar leve apresentaram 3 vezes (OR=3,02; IC 95%1,42-6,39) mais chances de desenvolverem sintomas depressivos, enquanto que aqueles em insegurança alimentar moderada/grave a chance foi 5 vezes maior (OR=5,01; IC 95% 2,30-10,92). Conclusão Foi encontrada associação entre insegurança alimentar e sintomas depressivos em domicílios com idosos da Estratégia Saúde da Família, com mais chances para aqueles com insegurança alimentar moderada/grave.

ABSTRACT

In April 2020, the COVID-19 pandemic became a severe threat to long-term care facility patients worldwide. A national front was urgently organized to integrate regional oversight and workgroups, coordinate activities, and develop educational materials, meetings, and communication strategies with these institutions in Brazil. As of August 2021, the front's initiatives have demonstrated its relevance for helping long-term care facilities cope with the COVID-19 pandemic, as well as for the ongoing struggle to include this issue in the public agenda, given that these facilities are an indispensable link in the development of a national policy for continuing care. This paper describes the history and initiatives of the National Front for Strengthening Long-Term Care Facilities regarding the COVID-19 pandemic, as well as its successful volunteer initiatives regarding the care of institutionalized older adults.


Em abril de 2020, a pandemia de COVID-19 trouxe graves ameaças de morbidade e mortalidade aos residentes de instituições de longa permanência para idosos (ILPI) em âmbito internacional. No Brasil, em caráter de urgência, fez-se então necessária a organização de uma frente nacional que integrasse, por meio de coordenações regionais e de grupos de trabalho, as diferentes ações e desenvolvimento de materiais educativos, lives e estratégias de interlocução com essas instituições. Com as produções feitas até agosto de 2021, constata-se a relevância do apoio às ILPI no enfrentamento da COVID-19, bem como da luta permanente por sua inclusão na pauta pública, como um imprescindível lócus do cuidado à pessoa idosa, atrelado à construção de uma política nacional de cuidados continuados. Apresentam-se neste artigo o histórico e a atuação da Frente Nacional de Fortalecimento às Instituições de Longa Permanência para Idosos (FN-ILPI) no enfrentamento da pandemia de COVID-19, além das ações exitosas de mobilização voluntária da sociedade civil no cuidado à pessoa idosa institucionalizada.

ABSTRACT

ABSTRACT OBJECTIVE To investigate the risk of mortality associated with sarcopenic obesity (SO), obesity (OB), and sarcopenia in elderlies. METHODS We analyzed longitudinal data from 270 participants > 65 years of age of Phase III of the Study on Frailty in Brazilian Older People (FIBRA-RJ-2012). Socioeconomic, demographic, lifestyle, morbidity, and functional data were collected by home based interviews. DXA and body composition assessment was conducted in a laboratory. In women, OB was diagnosed when body fat percentage ≥ 38% and sarcopenia by an Appendicular Lean Mass Index (ALMI) < 6.00 kg/m2 and muscle strength < 16 Kgf. In men, OB was diagnosed when body fat percentage ≥ 27%, and sarcopenia was diagnosed with ALMI < 7.00 kg/m2 and muscle strength < 27 Kgf. SO was assessed by combining variables used to diagnose obesity and sarcopenia. The probabilistic linkage method was used to obtain deaths in the 2012-January 2017 period from the Brazilian Mortality Registry. Cox regression models were tested, and crude and adjusted hazard ratio calculations were conducted. RESULTS After adjusting for sex, age, race/skin color, walking as an exercise, and hypertension, individuals with sarcopenia were 5.7 times more likely to die (95%CI: 1.17-27.99) than others without sarcopenia and obesity. CONCLUSION A high risk of death was observed in individuals with sarcopenia. These results show the need for preventive strategies of early detection and treatment in order to increase survival employing multimodal interventions.
